This information has been developed using findings of a national evaluation of hospice at home services in England, and offers practical, evidence-based recommendations. We have summarised what worked for Hospice at Home providers across the country when addressing challenges in the six key areas. Each area is further divided into four topics on commonplace challenges. Clicking on any of these topics will then take you to a list of suggestions for addressing the challengers. We know not all suggestions will be applicable to your situation or your service, but some will. The suggestions can be used to optimise Hospice at Home delivery.
